CHARLOTTE, N.C. — A North Carolina woman is speaking out about a part of her face she's been insecure about for decades.

She hopes to start a conversation about body image and finding self-love

Trinette Horner always has her phone at her side to post on social media. She poses for 10 to 200 pictures a day and makes a living off of it.

“I do photo shoots for a jewelry company, and I model for 10 different clothing brands,” Horner said. “I also do makeup and hair products.”

The reflection she sees in the mirror is one she smiles back at. But getting to the point where she is happy in her own skin has been a long journey. Horner said she was bullied as a child and those memories made her insecure into adulthood.

“They would call me a witch's nose, that was the biggest one,” Horner said.

It’s why she learned to do makeup and contour her nose to make it appear smaller. But her little imperfections still made her feel insecure.

“When I would contour, I would think, 'there is my witch's nose and that’s why it is so hard for me to do my make-up,'” Horner said. “In the back of my mind I would always replay those things that people would say to me.”

In her thirties she decided to make a change. After years of research she decided to go to Charlotte Plastic Surgery.

“People who didn’t know me said, 'Oh that is vain,' or, 'You are fake,'” Horner said. “To me it is not much different than coloring your hair or going to work out at the gym. You are kind of changing your appearance.”

Horner met with Dr. Stephan Finical and had rhinoplasty to reshape her nose. The surgery took two hours under anesthesia.

“She had her nose bridge taken down and her tip narrowed and rotated up,” Finical said. “It made her nose appear softer.”

Finical said he is often amazed by how a small procedure can make a big difference in how someone feels about themselves.

“A year ago I had a patient that I thought I just did a subtle operation on, and she said, ‘My daughter has never been more confident in her life, thank you,’” Finical said. “It really struck me because I thought I had done a pretty minor thing, but it really made a big impact on this lady's persona.”

Horner is now preparing for a wedding and recently had engagement photos taken. She said in the past she never allowed people to take side photographs because she didn’t like the way her nose looked. Now she is excited to take them. But she knows plastic surgery isn’t for everyone. She said people should do whatever makes them happy.

“Do it for yourself and really think about that decision,” Horner said. “But do it for you. You are beautiful, but if there is something you want to tweak, then there is nothing wrong with that.”

Case Study: Rhinoplasty for the Witch’s Nose Deformity

Background: Noses have an incredible variety of shapes and are the most diverse and complex structural feature of the face. With over a dozen different anatomic facets to them, it is no wonder how different one’s nose is from another. But despite this structural diversity, there are several features of a nose that are associated with good aesthetics and desireable proportions. A straight dorsal line, a nasolabial angle of 90 to 100 degrees and a tip that is fairly narrow are the foundations of an aesthetically pleasing nose.

There are some common aesthetic deformities of the nose that many people have. A dorsal hump or bump, a low hanging tip (nasoloabial angle less than 90 degrees) and a wide or fat tip are all common patient concerns. But when they are all grouped together, they create a classic appearing nose deformity that some patient’s refer to as a ‘witch’s nose’. While this is an extreme phrase to refer to a nose, it relates to one that has a dorsal hump, a low hanging tip and an overall profile convexity or curve to it. This is obviously a miniscule version of the nasal appearance of a classically depicted witch’s nose but the concept can be appreciated.

Case Study: This 37 year-old female wanted to change the shape of her nose which had bothered her since she was a teenager. She felt her nose was too big and looked like a ‘witch’s nose’. She didn’t like the hump in the upper half of the nose and felt the tip was too long and turned down. She particularly did not like the way her nose looked in the side view. She had no breathing problems through her nose.

In presurgical counseling, changes were made to her nose in photographs in the front, side and oblique views. Changes were made to determine whether she wanted a smooth or slight concavity to her dorsal profile and how much tip shortening and tip elevation she found was enough.

She had an open rhinoplasty done under general anesthesia.The superior septum and osteocartilaginous junction was taken down 3 to 4mms to make it straight. The remaining upper nasal bones was rasped until smooth and flush. The caudal end of the septum was obliquely trimmed as well as a cephalic trim of 5mms near the dome. A columellar strut was placed and double dome sutures used to support and reshape the tip. The upper lateral cartilages were resutured to the superior border of the septum without spreader grafts.

Her nasal splint and tapes were removed one week later. She had no bruising (due to osteotomies not being performed) and surprisingly much less swelling than anticipated. The early changes were very pleasing and her follow-up three months later did not disappoint. Her nose had much better proportions with a smooth dorsal line, a more open nasolabial angle and a narrower tip. Her nose no longer had a witchy appearance.

1) The ‘witch’s nose’ deformity is associated with any degree of a nasal hump and a down plunging tip.

2) The goal of rhinoplasty in the ‘witch’s nose’ is to remove the nasal hump and lift and shorten the nasal tip.

3) Open rhinoplasty allows the visibility to achieve a smooth dorsal line and a lifted but not over rotated nasal tip.

After consulting with E! Botched doctor Paul Nassif, Belinda finally heard the news she'd been waiting for as he told her he could reconstruct her nose, using part of her rib. Dr Nassif explained to Belinda why she had been suffering for so many years. 'When the surgeon removed the hump, the middle part of your nose collapsed in,' he said. Dr Nassif and his colleague Dr Terry Dubrow were stunned when Belinda revealed she had had at least 50 other consultations, but that every doctor had turned her down and refused to touch her nose. She said that her original doctor had told her he was a 'plastic surgeon, not a miracle worker'.
